Sperm DNA integrity is crucial for fertilization and development of healthy offspring. The spermatozoon undergoes extensive molecular remodeling of its nucleus during later phases of spermatogenesis, which imparts compaction and protects the genetic content. Testicular (defective maturation and abortive apoptosis) and post-testicular (oxidative stress) mechanisms are implicated in the etiology of sperm DNA fragmentation (SDF), which affects both natural and assisted reproduction. This book provides an overview of SDF types, origin and comparative analysis of various SDF assays while primarily focusing on the clinical indications of SDF testing, including four clinical cases where SDF testing played a significant role in improving fertility outcome.

Infertility is defined as the inability of a couple to bring pregnancy to term after a year of unprotected sexual intercourse. According to the WHO, 8–10% of couples experience infertility. This book overviews the development, structure, and mode of action of treatments for ovulation induction (OI) and controlled ovarian stimulation (COS) for assisted reproductive techniques (ART). For more than 30 years, human menopausal gonadotropins (HMG) have been applied in the treatment of human infertility; clinical implications include ovulation induction in clomiphene-resistant anovulatory women and ovarian stimulation in assisted reproduction techniques.

Background: Luteal phase support (LPS) in assisted reproduction cycles has been widely investigated in recent years. Methods: A questionnaire was sent by secure e-mail to 1,480 clinicians involved in ART worldwide; 148 clinicians from 34 countries returned completed questionnaires. Results: Progesterone support is usually started on the day of oocyte retrieval; 80% of clinicians applied vaginal progesterone only, 6% intramuscular, 5% oral progestin and 5% subcutaneous progesterone. Conclusions: Vaginal administration was the preferred route for luteal phase support; the emerging use of the oral route confirms the expected shift in clinical practice.

In Assisted Reproductive Technology (ART), oocyte retrieval is a vital step. Ultrasound-guided transvaginal oocyte retrieval is presently the standard operation for IVF treatment. Follicular flushing is considered to maximize the number of oocytes retrieved and thereafter improve the rate of IVF pregnancy. Several factors can affect egg collection and/or egg damage, including pump vacuum flow, velocity, needle bore size and length, follicle pressure and size, and collection techniques.

The ESHRE Special Interest Group in Embryology published the Atlas of Embryology in 2000 as a supplement to Human Reproduction, a valuable reference used by embryologists worldwide. This new edition completely updates the first edition and is published online for flexibility of revision and ease of consultation. Planned updates include chapters on cryopreservation and time-lapse monitoring. The edition includes almost 400 figures grouped in four chapters: the oocyte, the zygote, the cleavage stage embryo and the blastocyst.
